Fairfield Police Department responded to a vehicle accident at 1:09 p.m. Monday at the intersection of Main Street and Washington Avenue. Julie Anne Chapius, 46, of Keosauqua, was driving a 2007 Chevrolet Uplander van north on Main Street.
